How to report

If urgent

If there is urgent danger, call 000 first.

For non-urgent issues:

  1. Tell someone if they are present
  2. Submit the incident form: Report a hazard / incident

What to report

Please report:

  • Maintenance issues — Things that are broken or not working
  • Safety hazards — Anything that could cause injury
  • Equipment problems — Faulty equipment or missing items
  • Incidents — Accidents, injuries, or near-misses
  • Damage — Any damage to the space or equipment

Incident reporting

If an incident occurs (injury, accident, near-miss), please report it:

  • As soon as practical after the incident
  • Include details: what happened, when, who was involved, any injuries
  • Include any first aid provided

This helps us maintain a safe space and identify any issues that need addressing.

What happens next

We will review your report and take action as needed. For safety issues, we will prioritise a fix. For general maintenance, we will add it to our list.
